I have done it twice out of Ponce Inlet (Daytona) but have friends who have done it much more. Ed Dwyer is one of the pioneers of the fishery and is pretty much "the man" when it comes to fishing the Otherside. He fishes out of Port Canaveral. Our runs from Ponce Inlet were around 90 miles but it can vary probably anywhere from 70-100. Basically, you leave around 4 am so you are at the otherside at daybreak and use your radar to find the birds. Then you put your tuna lures way back and try to intercept the school. It can be hit or miss but when it's good, it is really good. Usually if you can't find the tuna you will come across some massive weedlines and still have a great day catching dolphin. They apparently see a lot more marlin over there than on our side of the stream. On my last trip out there in April we never really found the birds so we fished a big weedline and caught a white marlin in addition to dolphin. We are making the run on center consoles so we get out there pretty fast.
