Probably cause the bend of the hook was pulling against the belly of the Bally. After rigging, hold the leader in hand and pull the tail of the bally, if it pulls against the belly, it will most likely spin. If it does pull the belly, take a sharp fillet knife and cut a small slice in front of the bend. Repeat the process till it does not pull against the belly. This helps me to do this, try it and see if it works for you.

BALLYHOO PREP PRIOR HOOK PLACEMENT IS ESSENTIAL
I WOULD LIKE TO SEE A PIC OF ONE YOU HAVE RIGGED.....THEN YOU WOULD HAVE YOUR PROBLEM SOLVED I PROMISE....
TOUGH QUESTION HERE TO ANSWER WITH A KEYBOARD BRO
.....HANDS ON TUITORING WITH A MATE OR CAPTIAN BY YOUR SIDE WHO DOES THIS REGUALARLY WOULD BE HELPFUL
THE ANSWER GIVEN ABOVE IS VERY CORRECT....THE BALLYHOO SHOULD BE PULLED BY THE "BILL AREA " INSTEAD OF UNWANTED PRESSURE ON THE HOOK'S EXIT HOLE
WHAT SIZE AND TYPE OF HOOK DID YOU HAVE?

Sounds like all the hoo's I rig
1 thing you can do to help yourself out if your having problems is this;
Put the bait flat in your hand and lay the hook next to the bait where its going to be when your rig it inside the bait.
Mark the spot on the belly with your thumb where the bend of the hook is going to come out of the belly of the bait.
Then take the point of the hook and make a small hole in the center of the belly at the spot your have marked with your thumb.
When you do this your hook should come out in the right spot every time. It is more time consuming, but your hook placement should be correct each time.
If there is any tension on the front of the hook at the bend from the belly of the bait, it will spin like a helo.

No spin zone
Are you pin rigging or monel rigging? With monel I twist the monel to the hook leaving a 6 inch tag end and about 10 inches on the hook end. I then wrap the tag end around the crimp. After I insert the hook I push the tag end of the monel up through the "pin" spot and wrap the mouth closed. Then the back end of the monel goes around the gills, 2 wtraps behind the wieght, 2 wraps infront of the wieght and then twisted over the nose.
I thought about this rig for while before I decided to try it, and guess what? It worked.

Sit down Shut up And fish
I think a lot of guys mark the location of the hole where the hook shank starts it's bend. I use to make this mistake all the time. The hook is mostly likey sticking out to far. I agree cutting a slit will work but I don't like this as the bait will wash out faster.

In addition to making sure the bait id pulled from the head/bill and not the belly/hook bend, I'd gently squeeze the bait, breaking the bones to make it more limber and squeezing the poop and air bladder out the vent.
