Each manufacturer can vary a little, but I believe ring size is the inside diameter of the ring in mm. So a size 8 guide will be 8mm ID.
As for Box's question, I think that is up to the builder. Personally, I measure from ring to ring starting from the tip. On the lighter speck rods I build I usually have three inches between the tip and the first guide. That three inches is from the tip to the ring on the first guide, and then measure the rest as well from ring to ring.
I may be off a little, but those are my understandings of how it is done.
guides are usually sized in MM... Often times, you can find a guide spacing chart for the guides and blanks - most are measured from the tip top back.
I find that to be a decent starting point, and then will usually do a static test to make sure the spacing is good.

Guides for spinning rods that have a ceramic insert are measured from the OUTSIDE of the ceramic ring to outside on the other side. So, 12mm guide will have an opening of about 8mm or so.
