I've only reached the bottom of the spool a couple times in my life but in my limited experience, if the line broke it happens at the hook or leader, not at the spool. My understanding is that tension is (should be) greater at the hook end, mainly because the running fish creates extra drag on the other end because it's dragging all that line in the water. If you've ever let out half a spool while you're underway you know what I'm talking about.
Either way a fish that's in the ocean has a better chance of survival than one that's in my coolerI've caught plenty of otherwise healthy fish with hooks and leaders hanging out of their mouth or deep in their gut.
