Boldwater has always done good work for me. But, it you're looking for another good source for embroidered stuff, from t-shirts to fleece and jackets, try these guys. Queensboro

Just received a couple dozen denim shirts, tee's, fleece pullovers and a couple of different jackets and the quality is very good, the embroidery excellent... and the embroidery is free, even the set up is free as opposed to most companies that charge $50 to $80 bucks to prep the logo.
