Talk about not paying attention.
The larger vessel appears to be at fault for not giving way to a vessel crossing its bow from the starboard approach. It should have altered course to starboard and passed behind the smaller boat.
But a study of the wake of the smaller vessel suggests it was slowly turning into the larger vessel.
The large vessel was the main culprit but the smaller boat was contributory.
What do others think?

Fault with the big boat, negligence with the little one...
